Health check prior to boarding

What does the health checklist entail prior to boarding?  Do they temperature check everyone boarding the ship?  Is there a ship nurse if someone gets sick after leaving port?

It asks basic questions about how you are feeling.   They no longer temp check anyone at the port.

There is a medical facility on board with a licensed physician and nurse(s?).

I wouldn't even call it a health check anymore. It's two very basic questions about pregnancy and about having a general list of symptoms in the last 24 hours, many of which perfectly healthy people can have on any one given day, especially after traveling (dry air on flights, eating habits while traveling, etc).

Here's what they ask
