Contact the Better Business Bureau. They have helped me in situations in the past when I went through something similar with a different company. Probably wouldn't hurt to let Royal know you will be going that route as well to see if that will put a pep in their step. You've waited long enough for the refund.

Not just the street vendors.....we did Dunns River Falls on one of our cruises and we forgot our tip money on the bus. The guide followed us onto the bus and when we gave him the tip, he stood there wanting more. 
As far as street vendors go...we never make eye contact. You do that you are in some serious trouble. 

We'll never get off the ship in Costa Maya again unless we have an excursion that'll let us out without going through the shops.  That area was awful.

In Jamaica we had a couple vendors actually climb aboard our boat that returns us to the ship and tell us that we forgot to "Tip" our guide that took us up Dunn's River Falls. We informed him that we always tip and did not forget.  He argued with the group for several minutes and had to be told to leave the boat because we were leaving the Falls. 

From our personal experiences, Dunn's River Falls in Jamaica and taxi drivers in Nassau, Bahamas. (I don't need a ride to anywhere! I just got back from somewhere else on the island!)
After being to these two ports, I found that vendors in St. Thomas and St. Kitts don't seem to be pushy at all!

I would think he will need all 3 shots before 14 days out to be considered fully vaccinated. Whether he needs 1 shot or 100 the final dose would need to be given at least 14 days before boarding.
Per Royal's FAQ - All Royal Caribbean guests age 12 and older must present proof of full COVID-19 vaccination with the final dose of their vaccine administered at least 14 days before sailing. 
I would get him a PCR test. You don't want to risk the whole cruise over it.

No I don’t have experience with it. There’s another board that has a discussion about this exact thing and the general consensus was that you couldn’t do it. However, the post is years old and there were one or two people that said if you let them know in advance they can probably accommodate you. I just think it may be one of those things that depends. 

Hard to quantify, but I would vote for Jamaica.  Cozumel a close 2nd.

I second @Cruisin and Boozincall about cozumel. During our allure OTS sailing we were swarmed by people trying to get us to give us a ride to "their special beach with great beer and food" . I also found some of the vendors at Roatan a bit annoying. But Cozumel was definitely the worst. 

I hope she doesn't see this post and figure you out by your username! 

If you also stop at Labadee, the vendors on the back road (not the fixed price store) can be pushy as indicated by @waytoo.  The worst we experienced was in Turkey, so no worries on your cruise.  St. Thomas wasn't bad -- it's your choice to go to souvenir shops or not.  Can't comment on St. Kitts.  Had no problems in San Juan or St. Maarten.

Jamaica has the most aggressive street vendors IMO.  The vendors on Labadee are aggressive as well but they're easy to avoid.  St. Thomas and St. Kitts aren't a problem...or never have been, for me.

Cozumel they are awful as much as I’m not liking that port.

It alternates so if you can find a nearby cabin online and then look at a deck plan you can determine what your room will be.

Yes, the beaches and main pool are free so there are many people that do not spend money on an excursion here. It is up to you whether you want just a relaxing beach day or would rather spend money on an activity or cabana, etc.
