Kerry B
My wife and I will be traveling on Wonder OTS on April 8th. We are both vaccinated, but my wife was vaccinated under her maiden name before we got married. The name on her card doesn’t match her married name. Is this going to be an issue? CrznTxn
She may need to bring proof of her name when she received the vaccinations especially if her passport is in her married name. The people checking are particular about names matching. Same goes for the COVID test 2 days prior to sailing, make sure the name on the test exactly matches your ID. The last two sailings we have been behind folks who have had this issue. They straightened it out and boarded but it takes extra time and a visit from the supervisor to get it approved. Luckyjay
Is a QR code provided by a medical clinic accepted as proof of vaccination instead of the CDC Vax Card?
Pooch
The $64,000 question! Not sure they can scan a QR code at the terminal. Be prepared to open the full document yourself:

I fly a couple times a month and the airlines and customs are allways ok with a picture on the phone.
