Neesa Posted December 16, 2021 Report Share Posted December 16, 2021 We had nothing on back of card but our daughters did have it. We uploaded all and they were accepted just fine. Just make sure to bring card to port, it was checked there. Ours are from 2 different Walgreens yet they just hand filled the card out and both different, looks like a child's hand writing.
