No random testing occurs, at least not so far.

You could get called as a close contact and asked to take a test.  For example if your cabin attendant tests positive they could call on you to take a test.  If a family member in another cabin goes to medical and tests positive they could identify you as a close contact and ask you to test.  

If you have a kid in AO and another kid in their group in AO tests positive they could ask to test your kid as a close contact.  

It's unknown what rules may be created for unvaccinated adults as they look to open cruising beyond just vaccinated adults.  A specific destination could require they test unvaccinated before they are allowed to disembark for example.  That is unknown at this point.
