RVacationer Posted June 23, 2022 Report Share Posted June 23, 2022 We are currently going through this issue. I used on doctor an they didn't know what a CLIA number or lab certification number is. So, my wife used a different doctor at our hospital, and they didn't provide that number either. So, we called RC customer support and described the whole delima, and she said as long as it has everything else, your name, dob, test type, test result and dr office address and phone, then I had everything needed.
